For patients being discharged to a post-acute facility, our cohesive team can accurately and efficiently communicate patient data and information to maintain patient safety and help them get home as soon as possible.
For patients being discharged home, our case managers initiate follow-up phone calls to address concerns and navigate barriers on the patient’s road to recovery.
We provide medical services in nearly 100 skilled nursing and rehabilitation, long-term care, and assisted and independent living facilities offering a variety of services including wound care, physiatry, MAT/Suboxone services, and overnight telemedicine and telehealth visits.
Our vast experience has enabled our team of physicians and advanced practice clinicians to develop a high-quality care model that is collaborative, efficient, and customer-service driven. We partner with the clinical team at each facility to promote strong continuity of care, and we’re committed to delivering personalized, compassionate care to all patients and support to their families.
